Begin by completing the form below for your entire rental party.  You will be asked to provide a valid form of payment, but will not be charged until your rental fitting is complete.  Thank you for your understanding, and we appreciate your business!

SKIER TYPE I

Ski Cautiously

  • Prefer slower speeds.

  • Prefer cautious skiing on smooth slopes of gentle to moderate pitch.

  • Prefer lower than average release/retention settings.

  • Prefer an increased risk of inadvertent binding release in order to gain increased releasability in a fall.

  • Entry-level skiers uncertain of their classification.

SKIER TYPE II

Ski Moderately

  • Prefer a variety of speeds.

  • Prefer to ski on varied terrain.

  • Skiers not classified as Type I or Type III.

  • Prefer average release/retention settings appropriate for most recreational skiing.

SKIER TYPE III

Ski Aggressively

  • Prefer faster speeds.

  • Prefer fast and aggressive skiing on slopes of moderate to steep pitch.

  • Prefer higher than average release/retention settings.

  • Prefer decreased releasability in a fall in order to gain decreased risk of inadvertent binding release.

ACKNOWLEDGEMENT OF PERSONAL INFORMATION, EQUIPMENT INSTRUCTIONS & LIABILITY AGREEMENT

I have accurately represented the above listed information and it is true and correct. I will not use any of my equipment until I have received instruction on its use and I fully understand its use and function. I agree to verify that the visual indicator settings to be recorded on this form for downhill ski equipment agree with the number appearing in the visual indicator windows of the equipment to be listed on this form. I understand this user’s responsibility to securely wear the snowboard retention strap at all times; and I agree to verify that the bindings have been mounted and adjusted according to my instructions (snowboard only). I agree to read and execute the LIABILITY RELEASE AGREEMENT below.

EQUIPMENT RENTAL & LIABILITY RELEASE AGREEMENT


I understand that the binding system cannot guarantee the user’s safety. In downhill skiing, the binding system will not release at all times or under all circumstances where release may prevent injury or death, nor is it possible to predict every situation in which it will release. In snowboarding, cross-country skiing, skiboarding, snowshoeing and other sports utilizing equipment with non-release bindings, the binding system will not ordinarily release during use; these bindings are not designed to release as a result of forces generated during ordinary operation.

I understand that the sports of skiing, snowboarding, skiboarding, snowshoeing and other sports (collectively “RECREATIONAL SNOW SPORTS”) involve inherent and other risks of INJURY and DEATH. I voluntarily agree to expressly assume all risks of injury or death that may result from these RECREATIONAL SNOW SPORTS, or which relate in any way to the use of this equipment. I understand that a helmet designed for RECREATIONAL SNOW SPORTS use will help reduce the risk of some types of injuries to the user at slower speeds. I recognize that serious injury or death can result from both low and high energy impacts, even when a helmet is worn.

I AGREE TO RELEASE AND HOLD HARMLESS the ski/snowboard shop, its employees, owners, affiliates, agents, officers, directors, and the equipment manufacturers and distributors and their successors in interest (collectively “PROVIDERS”), from all liability for injury, death, property loss and damage which results from the equipment user’s participation in the RECREATIONAL SNOW SPORTS for which the equipment is provided, or which is related in any way to the use of this equipment, including all liability which results from the NEGLIGENCE OF PROVIDERS, or any other person or cause.

I further agree to defend and indemnify PROVIDERS for any loss or damage, including any that results from claims or lawsuits for personal injury, death, and properly loss or damage related in any way to the use of this equipment.

This agreement is governed by the applicable law of this state or province. If any provision of this agreement is determined to be unenforceable, all other provisions shall be given full force and effect.

BREAKAGE PROTECTION: This applies to equipment breakage. If the protection is accepted, the shop will absorb the cost of repair of any breakage to equipment. If declined, customer will pay for any breakage to equipment. However, accepting or declining protection does not apply to lost, stolen or abused equipment, which shall always be replaced by the renter at retail price. Breakage protection costs $3.00 per day for daily rentals and $15.00 for season rentals. Customer, BY INITIALS, accepts or declines coverage.

REFUNDS: There are NO REFUNDS on rental equipment for any reason (i.e. sickness, cancelled trips, etc.) Customer, BY INITIALS, accepts and understands refund policy.

SEASON RENTAL RETURN PERIOD: If you would like to return the equipment before the winter ends, they are due back NO LATER THAN FEBRUARY 28TH OF THE CURRENT RENTAL YEAR. Otherwise the equipment is due back from October 1st to November 15th in preparation for the following winter. If the equipment is returned any time outside of the rental return period, $50.00 EARLY TERMINATION FEE will apply as per rental agreement. If equipment is not returned at all, I authorize Hunterdon Ski and Snowboard to charge my card for the full retail value of any equipment rented and not returned. If the equipment is returned more than 30 days late, I authorize Hunterdon Ski to charge my card for a full rental year. Customer, BY INITIALS, accepts and understands seasonal lease period and charges.

DAILY LEASE: Additional daily rental fee will be charged if equipment is not returned by 12 PM (noon) on due date. Customer, BY INITIALS, accepts and understands daily lease period and charges.
